Century Homecare Ranked Top Homecare Provider in Central Massachusetts by Medicare

Worcester, MA — April 24, 2015 — Data released by Medicare ranks Century Homecare as one of the top performers in the state and 2nd in Central Massachusetts. Century Homecare’s 13% re-hospitalization rate for the 4th quarter of 2014 was well below the state average of 17%. Medicare continuously tracks re-hospitalization rates by provider and releases quarterly rankings.

Hospital readmission figures are a closely watched measure of performance. Many hospitals and inpatient facilities incur financial penalties when their patients are readmitted soon after discharge. Hospitals rely on homecare providers like Century to provide quality care at home and prevent re-hospitalization.

“This is wonderful news, not only for Century Homecare, but also for our patients and their families,” said William von Rolappe, Director of Quality Assurance and Compliance at Century Homecare. “Reducing re-hospitalization is our top priority and the center of our care planning process. Such a low number of hospitalizations is particularly impressive for Century because we serve a large population of mental health patients. These patients are at the highest risk for readmission partly due to complications with medications and social and economic factors including homelessness and substance abuse.”

“From the day we receive a patient from the hospital, our goal is to keep them safe and healthy in their own home,” said Elizabeth Ciera, Assistant Administrator. “We identify what we call ‘barriers to compliance’ and employ a multi-disciplinary team of clinicians to address these barriers. Our nurses and therapists provide the necessary treatment while our case managers help with non-medical needs such as arranging for transportation and setting up follow-up appointments.”

“According to the latest data from Medicare, 30% of all hospital admissions are the result of medication errors.” said Dr. Njoroge, the Chief Clinical Officer at Century. “By providing our patients with a variety of options to manage their medications, we ensure that they stay compliant. Re-hospitalization is very disrupting to patients and their families and can lead to further complications especially for elderly patients. We welcome this good news and will continue to work with our staff to bring the number down to zero.“

Century Homecare is nationally accredited by Community Health Accreditation Program (CHAP) and is certified by both Medicare and Medicaid. They provide a comprehensive range of homecare services including medical and psychiatric care at home. Century’s corporate office is in Worcester with additional branches located in Holyoke and Brockton. Together, Century’s branches provide patient care throughout the state of Massachusetts. Their staff of over 500 includes nurses, physical therapists, occupational therapists, speech therapists, respiratory therapists, social workers and home health aides. A clinical pharmacist and a psychiatrist are also on staff.

Other local home care companies highlighted in the ranking include: Worcester VNA with 16%; Overlook VNA with 18%; Acclaim Home Healthcare with 20%; Metrowest Homecare with 17% and Epic Health Services with 24%.
